 The type of personal information we collect 

We currently collect and process the following information:

  • Personal identifiers, contacts and characteristics (for example, name and contact details)
  • Some personal health information
  • Information about your GP 
  • Information (name and contact details) about someone who supports you.

How we get the personal information and why we have it

Most of the personal information we process is provided to us directly by you for one of the following reasons:

  • To get in touch to make appointments.
  • To keep in touch about upcoming events or information for people who have completed work with Beyond Therapy.
  • To book 1-1 or group sessions.
  • To aid personal safety – some information is collected to ensure that if you became unwell during a session or if there were immediate concerns about your welfare contact could be made with appropriate professionals.

We use the information that you have given us in order to make appointments, send over additional information via email if agreed, get in touch with individuals or other professionals if there are immediate concerns about your welfare, and keeping up to date notes to support your treatment. These are only visible to Hannah Bailey.

 

We may share this information with your GP or emergency services if there are immediate concerns for your wellbeing. Information is not shared with anyone else. 

 

Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are:

(a) Your consent. You are able to remove your consent at any time. You can do this by contacting Hannah Bailey (hannah@beyondtherapysuffolk.co.uk / 07570985208)

(b) We have a legal obligation.

(c) We have a vital interest.

How we store your personal information 

Your information is securely stored. 

We keep personal contact details for 12 months since our last contact (in person, by phone, email, or instant message). We will then dispose your information by deleting these from our computer systems. These are kept in a database of previous clients and will be removed once 12 months has passed.

We keep notes and assessments letters for 1-1 clinical sessions for 7 years following the final session. These are not kept in a paper format and will be deleted from the server when no longer required. 

 

Your data protection rights

Under data protection law, you have rights including:

Your right of access - You have the right to ask us for copies of your personal information. 

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete. 

Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances. 

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances. 

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
